Open lines of communication are one of the many keys to success and we will do what is necessary on our part to ensure that the communication is clear regarding the treatment and progress of your patient.
Please complete the referral form (below) and fax it to
(912) 544-0864. Once we receive the form, we will contact your office to acknowledge receipt of the referral and then contact the patient to schedule the appointment.
We will either mail the patient a New Patient Package or have the patient download the information from our website — whichever is most convenient for the patient.
